💢 Many Young Females Experience Perimenopausal Symptoms

A recent study published in npj Women's Health found that many women aged 30-35 may be experiencing untreated perimenopausal symptoms.

The study analyzed self-reported data from over 4,400 women and found that 55.4% of women in this age group reported moderate to severe perimenopausal symptoms. However, most women did not seek treatment for their symptoms until they were 56 or older.

The study also found that women in this age group were more likely to experience psychological symptoms such as depression, irritability, and anxiety, which tended to occur before physical symptoms.

Many women assume that menopause is a 50s problem, and therefore, dismiss their symptoms as stress, aging, or lifestyle factors. However, perimenopause can start as early as the early to mid-30s, and ignoring symptoms can lead to unnecessary suffering.

The study's authors suggest that hormone replacement therapy (HRT) can be a helpful resource for some women, but it is essential to see a practitioner who understands the intricacies of hormone therapy.

Lifestyle changes, such as getting adequate sleep, staying hydrated, eating healthy foods, and engaging in gentle exercise and stress-reducing activities, can also help alleviate symptoms.

Additionally, evaluating and treating the gut microbiome, as well as using lubricants, moisturizers, and pelvic floor exercises, may provide relief for some women.

Overall, the study's findings emphasize the importance of recognizing and addressing perimenopausal symptoms, regardless of age. By seeking help and making lifestyle changes, women can find relief and improve their overall quality of life.

💢 Spicy food consumption reduces the risk of ischemic stroke: a prospective study

British Journal of Nutrition. 2024 Jan 30:1-25.

📊 Previous studies suggested a link between consuming spicy food and reduced mortality from cardiovascular disease, yet the relationship with stroke types and dose-response remained unexplored.

🔍 The study involved 50,174 participants aged 30-79 years, collecting data on spicy food consumption through baseline surveys. Over a median 10.7-year follow-up, 3,967 strokes were recorded, including 3,494 ischemic strokes and 516 hemorrhagic strokes.

🌶 Compared to those who rarely consumed spicy food, participants consuming spicy food 1-5 days/week showed reduced risk of overall stroke and ischemic stroke.

⚖️ The analysis revealed a U-shaped dose-response relationship, indicating that moderate consumption of spicy food was associated with the lowest risk of ischemic stroke. Notably, obese participants consuming spicy food ≥3 days/week showed a notably reduced risk of ischemic stroke, suggesting a potential protective effect in this subgroup.

📑 While further research is needed to elucidate the underlying mechanisms and validate these findings in diverse populations, this study provides valuable insights into the role of spicy food in mitigating stroke risk.

💢 Five Things Clinicians Need to Know About Zoonotic Viral Spillover and Spillback

AMA Journal of Ethics

Since the 1990s, infectious diseases originating in animals have posed a significant threat to global health. Despite efforts, current surveillance and mitigation strategies have been criticized.

🦠 Lesson 1: Viruses Are Everywhere
The vast number of diverse viral threats, both externally and within the human body, highlights the remarkable effectiveness of the human immune system in preventing viral diseases.

🧪 Lesson 2: Diagnostic Challenges
Modern diagnostic tools, while sensitive and specific for known viruses, can miss clinically important viral infections. Delays and challenges in developing diagnostics for emerging viruses underscore the need for continuous improvement in laboratory diagnostic science.

🌍 Lesson 3: Viral Spillover Variability
An estimated 1.67 million undiscovered viruses are present in mammal and bird hosts. Detecting pathogens in early spillover stages requires specialized surveillance strategies and diagnostic approaches to prevent potential epidemics.

🔄 Lesson 4: Global Threat of Viral Spillback
Zoonotic viruses causing human diseases can spill over into other animal populations, mutate, and spill back, leading to additional outbreaks. Concerns about novel variants of SARS-CoV-2 in diverse animal hosts highlight the global and domestic public health threats posed by viral spillback events.

🩺 Lesson 5: Clinicians' Role and Responsibilities
Clinicians are urged to be knowledgeable and vigilant agents in detecting spillover events. Recognizing the limitations of current detection methods, clinicians should actively engage with public health officials and contribute to reporting networks to facilitate timely responses to potential outbreaks.

In conclusion, the article emphasizes the need for a comprehensive, multidisciplinary approach, incorporating One Health principles, to enhance the detection and response capabilities for emerging viral pathogens. Clinicians play a crucial role in this process, necessitating ongoing education and collaboration to effectively address the evolving landscape of viral zoonoses.

💢 Coffee Boosts Brain Function and Makes You Live Longer

While we always want to be careful of the risk of confusing correlation for causation, the volume and consistency of these studies is compelling. Here's a quick look back at a few of the most persuasive and interesting among them:

A double-blind study out of Johns Hopkins University a decade ago and published in the journal Nature Neuroscience found that giving study participants a 200-milligram caffeine tablet was correlated with better performance on memory tests. "We've always known that caffeine has cognitive-enhancing effects," said the lead author.

A massive study of nearly 500,000 British adults published in JAMA Internal Medicine found that those who drank at least two to three cups of coffee per day -- and as many as eight cups -- were "about 10 percent to 15 percent less likely to die" of any cause during the 10-year study period than those who didn't.

A study of the coffee habits of 12,583 people in the Journal of the American Medical Directors Association, found that those who drank four or more cups of coffee per day were doubly likely to avoid physically frailty into their 70s.

A Canadian study published in 2017 the journal Frontiers in Neuroscience found that phenylindanes, a chemical created during the coffee roasting process, seems to stop the buildup of two toxic proteins in the brain, which have been linked to Alzheimer's and Parkinsons's diseases. "Coffee consumption does seem to have some correlation to a decreased risk," said one of the study authors. "But we wanted to investigate why."

A study last year in the journal GeroScience found that mice who given trigonelline (TG), which is a plant alkaloid found in coffee, performed better in experiments testing their "acquiring, retaining, structuring, and applying information" abilities.

Another study, published in the Annals of Internal Medicine, examined the habits of nearly 350,000 cpeople, and found that the moderate to heavy coffee drinkers among them were 30 percent less likely to die of any cause during the study period than those who did not.

A study in which researchers were funded by the American Heart Association and the University of Colorado School of Medicine found that risk of heart failure or stroke went down 8 percent for each additional cup of coffee per day.

We can keep going -- and I'm sure I will, because as a copious coffee drinker myself, I'm always on the look for these kinds of studies.

As written in the free ebook, Neuroscience: 13 Ways to Train Your Brain for Life, sometimes the best health news is when you learn that something you like to do anyway is even better for you than you realized.

💢 Is Semaglutide Use Associated with Nonarteritic Anterior Ischemic Optic Neuropathy?

Nonarteritic anterior ischemic optic neuropathy (NAION) is a rare cause of optic neuropathy that can result in blindness. A recent study suggested an association between NAION and glucagon-like peptide-1 (GLP-1)–receptor agonist semaglutide in an ophthalmology referral population.

To examine a potential association in the general population, researchers evaluated 14 U.S. databases with information on 37 million patients with type 2 diabetes who used semaglutide or other common diabetes medications. Two study designs were used to help limit confounding: A new-user cohort (800,000 patients) with propensity-score matching and a self-controlled case series (40,000 patients).

In the propensity-score analysis, semaglutide was associated with significant excess risk for NAION compared with empagliflozin (hazard ratio, 2.3). In the self-controlled case analysis, semaglutide and exenatide exposures were associated with a significantly higher incidence of NAION (HRs, 1.5 and 1.6, respectively). The NAION incidence rate among new semaglutide users ranged from 9 to 15 per 100,000 person-years, depending on the definition used to identify NAION.

This study adds to the growing evidence of an association between semaglutide and NAION; similar findings were reported in a recent population-based Danish study. Although this body of observational research doesn't prove causality, informing patients about this rare potential complication when initiating semaglutide seems prudent. The mechanism remains unclear, but editorialists note the presence of GLP-1 receptors in retinal ganglion cells, which form the optic nerve.

Low-dose aspirin and prevention of colorectal cancer: evidence from a nationwide registry-based cohort in Norway. Am J Gastroenterol. 2024 Feb 1

HR=0.91 (95% CI 0.86-0.95) for <3 years,
HR=0.85 (0.80-0.91) for 3-5 years, and
HR=0.84 (0.80-0.88) for ≥5 years, compared to never use.

💢 Risk for CKD, CVD Lower for Adults With Evidence of
Type 2 Diabetes Remission

📊 A recent study, published in Diabetologia, reveals that individuals showing evidence of remission from type 2 diabetes experience a significant reduction in the risks associated with chronic kidney disease (CKD) and cardiovascular disease (CVD). The study, led by Edward W. Gregg, Ph.D., from the Royal College of Surgeons of Ireland in Dublin, involved a multicenter randomized controlled trial comparing a 12-year intensive lifestyle intervention with diabetes support and education for long-term outcomes.

🩺 The incidence of CVD and CKD was compared among 4,402 and 4,132 participants based on the achievement and duration of diabetes remission. Cardiovascular disease incidence was defined broadly, including nonfatal acute myocardial infarction, stroke, admission for angina, or CVD death. High-risk or very high-risk CKD categories were determined using the Kidney Disease Improving Global Outcomes criteria.

📉 The results, after adjusting for various factors such as hemoglobin A1c, blood pressure, lipid levels, CVD history, diabetes duration, and intervention arm, showed that participants with evidence of any remission during follow-up had significantly reduced risks for CKD and the composite CVD measure (hazard ratios of 0.67 and 0.60, respectively). Interestingly, individuals with longer-term remission demonstrated the greatest magnitude of risk reduction.

🌟 Edward W. Gregg highlighted the study's significance, stating that it is the first intervention study linking remission with a reduction in diabetes-related complications. He expressed optimism, calling it encouraging news for those who can achieve remission from type 2 diabetes. Despite acknowledging the challenges of maintaining weight loss and remission, Gregg suggested that any success with remission is associated with later health benefits. The findings emphasize the potential positive impact of achieving remission on long-term health outcomes for individuals with type 2 diabetes.
