A previously healthy 25-year-old man presented to the general medicine clinic with a 3-month history of worsening fatigue, nausea, and dry cough. One week before the onset of his symptoms, he had injected 4 ml of elemental mercury intravenously as a folk remedy for tinea cruris. Conjunctival pallor was observed on physical examination. Laboratory studies showed a hemoglobin level of 6.5 g per deciliter (reference range, 12.0 to 16.0), a serum mercury level of 430 μg per deciliter (21,440 nmol per liter; reference value, <1.5 μg per deciliter [70 nmol per liter]), a 24-hour urine mercury level of 29,792 μg (148,510 nmol; reference value, <45 μg [220 nmol]), and normal renal function. A chest radiograph (Panel A) showed diffusely distributed metallic opacities in the lungs, right atrium (arrow), and right ventricle (asterisk). An abdominal radiograph (Panel B) showed similar metallic densities in the liver, kidneys, iliac veins, and left gonadal vein. A diagnosis of mercury poisoning was made. Inpatient chelation therapy with intravenous unithiol was initiated. After 2 weeks of treatment, the patient’s symptoms had abated, his hemoglobin level had increased to 8.0 g per deciliter, and his serum mercury level had decreased to 254 μg per deciliter (12,660 nmol per liter). The patient was discharged from the hospital with a recommendation to continue chelation therapy with oral dimercaptosuccinic acid.

NICE has recommended three drugs—nirmatrelvir plus ritonavir (Paxlovid), tocilizumab (RoActemra), and baricitinib (Olumiant)—for the treatment of covid-19 in adults, as part of draft guidance.

NICE has not recommended other covid-19 treatments, including casirivimab plus imdevimab (Ronapreve), molnupiravir (Lagevrio), remdesivir (Veklury), sotrovimab (Xevudy), and tixagevimab plus cilgavimab (Evusheld).

💢 داروهای در دست مطالعه برای کبدچرب که امریکا، اتحادیه اروپا و ژاپن


تا امروز (25 آبان 1401) حتی یک داروی مورد تأیید FDA برای درمان کبدچرب وجود ندارد. تنها درمان کبدچرب تا امروز، تغییر شیوه زندگی و در محوریت آن، کاهش وزن است.

همانطور که ملاحظه می کنید تعداد زیادی دارو اعم از خوراکی و تزریقی در مراکز متعدد علمی دنیا و توسط شرکت های متعدد داروسازی در حال تحقیق و توسعه هستند.

وقتی یک چهارم جمعیت جهان در شرق و غرب عالم به کبدچرب دچار هستند، چنین پدیده ای اصلا عجیب نیست.

عجیب آن است که مدعیان دروغین طب سنتی و گیاهی و علفی و ... هنوز هم دست بر نمی دارند و با فروختن محصولات عجیب و غریب به اسم درمان کبدچرب، جیب خلق الله را با روشهای مختلف خالی می کنند.

🔹دیابت بارداری زمانی روی می دهد که بدن مادر باردار نتواند به میزان مورد نیاز انسولین تولید کند.

🔹یک وضعیت موقتی و محدود به دوران بارداری است.

🔹خطر عوارض بارداری و زایمان را افزایش می دهد.

🔹خطر دیابت نوع 2 را در مادر و فرزند در آینده افزایش می دهد.

🔹بیعلامت است و در غربالگری های بارداری تشخیص داده می شود.

🔹قابل پیشگیری نیست.

🔹By definition, internal hemorrhoids are proximal to the dentate line and covered by insensate anorectal mucosa. As such, internal hemorrhoids may prolapse and bleed but are rarely painful.

🔹Internal hemorrhoids are graded by the degree of prolapse. First-degree hemorrhoids prolapse beyond the dentate line when straining. Second-degree hemorrhoids prolapse through the anus upon straining but spontaneously reduce. Third-degree hemorrhoids prolapse through the anus upon straining and requires manual reduction. Fourth-degree hemorrhoids prolapse through the anus upon straining but cannot be manually reduced.

🔹External hemorrhoids occur distal to the dentate line, covered by richly innervated anoderm. As such, thrombosis of external hemorrhoids can yield significant pain. Adequate analgesia is paramount if ligation or excision is to be performed.

🔹First, second, and some third-degree internal hemorrhoids can benefit from minimally invasive, in-office procedures, that may spare a patient an operative hemorrhoidectomy. These procedures include rubber band ligation, sclerotherapy, and infrared photocoagulation.

🔹دیابت نوع 2 اغلب در بزرگسالی تشخیص داده می شود.

🔹در نتیجه مجموعه ای از عوامل شامل سابقه خانوادگی و عواملی چون رژیم غذایی نامناسب، فقدان فعالیت بدنی و مصرف دود ایجاد می شود.

🔹علایم: ادرار مکرر، تشنگی، کاهش وزن، خستگی. نکته مهم این که اغلب بیعلامت است و با انجام آزمایشات تشخیص داده می شود.

🔹درمان: مصرف منظم داروهای تجویز شده، کنترل قند خون، افزایش فعالیت بدنی، رژیم غذایی سالم، پیشگیری زودرس و شناسایی بموقع عوارض.
