في الأدوية، كلمة "كورتيزون" تطلق أحياناً على أدوية مثل Prednisone أو Hydrocortisone
الكورتيزول = الشكل النشط، مسؤول عن توازن الجسم أثناء الضغط
الكورتيزول هو الهرمون الطبيعي الأساسي الذي تفرزه الغدة الكظرية (Adrenal gland) ويعرف بـ "هرمون التوتر"
الكورتيزون = الشكل الأقل نشاطاً و يستخدم دوائياً
🩺 الأدوية مثل Hydrocortisone تماثل الكورتيزول، أما Prednisone يحتاج لتحول داخل الجسم
استخدام الستيرويدات لفترة طويلة بدون مراقبة = خطر تثبيط الغدة الكظرية..

Propofol Infusion Syndrome (PRIS):

العناية بمستوى البوتاسيوم قبل الإنسولين

في حالات الحماض الكيتوني السكري (DKA):
🔴 ممنوع إعطاء الإنسولين إذا كان مستوى البوتاسيوم في الدم أقل من 3.3 mEq/L
⬅️ لأن الإنسولين يُحفّز دخول البوتاسيوم إلى داخل الخلايا، مما يزيد من خطر حدوث نقص بوتاسيوم حاد (severe hypokalemia) وقد يكون ذلك مميتاً fatal نتيجة اضطرابات قلبية خطيرة..

الإجراء الصحيح:
- قبل البدء بالإنسولين، إذا كانت [K⁺] < 3.3 mEq/L:
- أعط Potassium Chloride بجرعة 20–40 mEq/hour IV
- استمر في المعايرة والمتابعة حتى يصل البوتاسيوم إلى ≥ 3.3 mEq/L
- عندها فقط يمكن بدء العلاج بالإنسولين..
🔴 في جديد الادوية أعلنت إدارة الغذاء والدواء الأمريكية (FDA) عن الموافقة على WIDAPLIK (مركب من Telmisartan, Amlodipine, Indapamide) لعلاج Hypertension لدى البالغين، ويشمل ذلك استخدامه كـ initial therapy للمرضى الذين يحتاجون إلى أكثر من دواء واحد منذ بداية العلاج.

🍃 امرأة تبلغ من العمر 64 عام، لديها تاريخ معروف بـ قصور القلب الانقباضي (HFrEF)، كانت حالتها مستقرة على العلاج. حضرت الآن إلى العيادة تشكو من:
ضيق تنفس عند الاستلقاء (Orthopnea) بدأ منذ أسبوعين ،
زيادة في الوزن بمقدار 2 كغ ،
تورم في الكاحلين ،
سعال خفيف ليلي..
🍃 الأدوية الحالية:
Lisinopril 10 mg يومياً
Furosemide 20 mg يومياً

Method of palpation of radial artery.
The radial pulse is felt using 3 fingers. The distal finger is to prevent the backflow, proximal finger is to stabilize artery on the bone and middle finger is used to feel and count the pulse (3-finger method).
