🔹 ECG may reveal changes produced by acute pericarditis. This test rarely reveals tamponade but is useful to rule out other cardiac disorders.

🔺Surgery: pericardiocentesis (needle aspiration of the pericardial cavity) or surgical creation of an opening to drain fluid, thoracotomy
🔺 Adrenergic agent: epinephrine
🔺 Inotropic agent: dopamine
🟪🔑 Key interventions
📣 If the client needs pericardiocentesis 👇
● Keep a pericardial aspiration needle attached to a 50-ml syringe by a three way stopcock, an ECG machine, and an emergency cart with a defibrillator at the bedside. Make sure the equipment is turned on and ready for immediate use.
● Position the client at a 45- to 60-degree angle. Connect the precordial ECG lead to the hub of the aspiration needle with an alligator clamp and connecting wire. When the needle touches the myocardium during fluid aspiration, an ST segment elevation or premature ventricular contractions appear.
● Monitor blood pressure and central venous pressure (CVP) during and after pericardiocentesis to monitor for complications such as hypotension, which may indicate cardiac chamber puncture.
● Watch for complications of pericardiocentesis, such as ven- tricular fibrillation, vasovagal response, or coronary artery or cardiac chamber puncture.
📣 If the client needs a thoracotomy 👇
● Explain the procedure to the client. Tell him what to expect postoperatively (chest tubes and chest tube drainage system, administration of oxygen). Teach him how to turn, deep-breathe, and cough.
● Maintain the chest tube drainage system and be alert for com- plications, such as hemorrhage and arrhythmias.

🔹ا ECG ممکن است تغییرات ناشی از پریکاردیت حاد را نشان دهد. این تست به ندرت تامپوناد را نشان می دهد اما برای رد سایر اختلالات قلبی مفید است.

🔺جراحی: پریکاردیوسنتز (اسپیراسیون حفرهی پریکارد با سوزن) یا ایجاد دهانهای برای تخلیه مایع، توراکوتومی
🔺 عامل آدرنرژیک: اپی نفرین
🔺 عامل اینوتروپیک: دوپامین
🔑 مداخلات کلیدی
📣 در صورتی که مراجعه کننده نیاز به پریکاردیوسنتز داشته باشد 👇
● یک سوزن آسپیراسیون پریکارد را با یک سه راهی به یک سرنگ 50 میلی لیتری متصل کنید ، یک دستگاه ECG و یک ترالی اورژانس با دفیبریلاتور در کنار تخت قرار دهید. مطمئن شوید که تجهیزات روشن و آماده برای استفاده فوری است.
● بیمار را در زاویه 45 تا 60 درجه قرار دهید. لید پره کوردیال ECG را با یک گیره الیگاتور به هاب سوزن آسپیراسیون وصل کنید. هنگامی که سوزن در حین آسپیراسیون مایع، میوکارد را لمس می کند، بالارفتن ST یا انقباضات زودرس بطنی (pvc) ظاهر می شود.
● فشار خون و فشار ورید مرکزی (CVP) را در حین و بعد از پریکاردیوسنتز کنترل کنید تا از نظر عوارضی مانند افت فشار خون که ممکن است نشان دهنده سوراخ شدن اتاقک قلب باشد، نظارت کنید.
● مراقب عوارض پریکاردیوسنتز، مانند فیبریلاسیون بطنی، پاسخ وازوواگال، یا سوراخ کردن عروق کرونر یا اتاقک قلب باشید.
📣در صورت نیاز بیمار به توراکوتومی👇
● روش کار را برای بیمار توضیح دهید. به او بگویید که بعد از عمل چه انتظاری داشته باشد (سیستم چست تیوب و سیستم تخلیه لوله سینه، تجویز اکسیژن). به او یاد دهید که چگونه بچرخد، نفس عمیق بکشد و سرفه کند.
● سیستم تخلیه لوله سینه را حفظ کنید و نسبت به عوارضی مانند خونریزی و آریتمی هوشیار باشید.
